Course overview


This two day course will help you understand and apply the ICAM root cause investigation methodology which is based on the Reason Swiss Cheese model.
It is designed for safety, operational, maintenance and supervisory personnel from all industry sectors who may currently participate in low to medium level investigations and
aim to become investigation leaders and involved in more complex events.
This course introduces best practice in incident investigation and develops practical skills which you can apply back in your workplace

Course outcomes

This course will help you answer the following questions.

  • Why do we carry out investigations?
  • What should we do immediately after an event?
  • What level of investigation do we need and who needs to be in the investigation team?
  • How do we go about gathering data, interviewing witnesses and examining evidence?
  • How do we establish the sequence of events?
  • What are the principles of ICAM?
  • How can we analyse the information using ICAM?
  • Can we get to the root causes of incidents?
  • What are the corrective actions and how do we ensure they get done?
  • How do we capture our findings in an investigation report?
